In-Home Supplemental Services
At times, patients receiving in-home nursing and/or rehabilitation services require additional support to recover and recuperate. RVNAhealth offers the following supplemental services to round out our clinical care and create an environment for recovery and health. (These are not stand-alone services, but are intended to support in-home skilled nursing and rehabilitation.)
In-Home Supplemental Services include:
CNAs: To help with personal care, such as bathing, dressing, toileting and other essential activities until an individual regains independence in these areas.
Discharge Planning and Care Coordination: These services ensure that patients have the support and environment they need to recover health and get stronger. Our medical social worker helps patients and families understand and secure the resources available to them.
Medical Nutrition Therapy: When conditions warrant, our registered dietitian provides nutrition counseling and dietary guidance to optimize recovery and health and address condition-specific needs.
