Today, RVNAhealth continues to offer in-home nursing services to 35+ communities across Connecticut. In-home nursing services include care management, symptom management, and pain management.
RVNAhealth nurses provide ongoing health monitoring, medication reconciliation, transitional care, and chronic disease management. Additionally, our clinicians specialize in Wound Care, Cardiac Care, Diabetes, and Palliative Care.
In 2018, RVNAhealth cared for over 2,000 patients in their homes
with over 43,000 individual visits.
